GP Connect
Cannock North PCN and its member GP Practices use GP Connect to make appointments at the Out of Hours Hub.
GP Connect is a service that enables authorised Out of Hours Doctors and Advanced Clinical Practitioners to quickly and efficiently and in real time, view your full GP medical records from your own GP Practice at your Out of Hours appointment This includes medications, referrals and other key clinical information to make a better and more informed decision about your treatment.
Third Party Access
If you require Third Party access to another persons records you need to obtain prior authorisation. You will need to come to the surgery in person along with the person you are requesting access for and complete the form below, or ask for the Receptionist for a Third Party Access Form.

Freedom of Information Act
The Freedom for Information Act 2000 gives the public the right to know how public services are run and how much they cost. From January 2005 General Practices must respond to requests about information it holds. However requests will have to satisfy a number of conditions. For further information please contact our Practice Manager.
Summary Access Requests (SARS)
We do not supply insurance companies with copies of your medical records. However we can provide you with access to your full medical record using Patient Access. Please see How to Register for Patient Access for further information.
NHS Summary Care Records
As part of a mandatory, national programme each GP Practice will have to make a summary care record for each patient (unless the patient has already opted out).
Please read the information regarding this.

Transferring Your Electronic Health Record
Your GP practice holds copies of your patient health record electronically and in paper format. Both contain the healthcare information about you that your GP needs including your medical history, medications, allergies, immunisations and vaccinations.
If you have previously registered with a different GP in England, upon registering at this practice your electronic health record will, where possible, be transferred automatically from your previous practice through the use of an NHS system called GP2GP.
