How to Register at our practice

To access the online healthcare record of another patient, you must complete a form. Start by downloading the Proxy Online Access to Health Record Request Form (in Microsoft Word format). Fill out the form and submit it to reception to complete the process.

If you do not wish to Register Online you can download and complete the following forms:

  • Proof of Identification (ID) (for example passport, driving licence or medical card [you may be asked for two forms of ID])
  • Proof of Address (e.g. utility bill or bank/credit card statement from the last four months, current tenancy agreement, government agency letter, higher education letter or a letter from a recognised employer).

You need to register your new baby at the practice as soon as possible. Therefore please complete a Register with a GP Surgery (PRF1) form (also available from reception) and send it to your surgery.   

To register a new baby you will require to show the Red Book or the Birth Certificate.  This also has to be registered by a named parent/carer or legal guardian (to provide their ID when registering).

We will require the child’s NHS Number. Once we have received the form we will register your baby as a patient at this Practice.

The Practice will arrange an appointment for a mother and baby post-natal check. This is a 30 minute appointment. Your baby’s course of routine immunisations will start at 8 weeks and you will receive an appointment for this. 

Please remember to bring the red book. If you cannot make this appointment you must notify reception as soon as possible. Please be advised that if you do need to cancel, the next available appointment may be anything up to 2 weeks after that date.

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required.

In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport plus download and complete a Temporary Resident Registration form (also available from reception).

An NHS number is a 10 digit number, like 485 777 3456.

Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.

Accountable Named GP for All Patients

All patients registered at the Hednesford Medical Practice have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
